In it’s initial year the program was able to collaborate with local Family Health Teams and organizations, hospitals and home and community care support services to help 300 patients in the community.
The outcomes the County says have had system-wide impacts and have provided significant reassurance to patients, their families and caregivers.
To become part of the Community Paramedicine Program contact your Primary Care Provider or Home or Community Care Support Service to review eligibility criteria.
If you don’t have any of those contact 519-881-1291 and ask to speak to the Community Paramedicine Team Assistant.
