In a news release, the new health team is a new approach to health care bringing together health care providers as one collaborative team.
They say this will ensure patients will experience easier transitions with one patient story, one patient record and one care plan that is shared between the health care providers.
The eight new health teams brings the total to 50 covering 92 per cent of the province’s population at maturity.
