Bruce County shared word of its Community Paramedicine Program, which was launched back in January 2021, and provides palliative care, wellness checks, remote care monitoring, CP Clinics, 911 referrals, outreach and more.
It serves the interior, lakeshore, and northern municipalities.
24 Bruce County Paramedic Services staff are trained as Community Paramedics and the robust training plan continues to respond to the needs and gaps identified in the community and the healthcare system.
The team undertake an average 12 to 20 visits per day.
They have 970 patients enrolled in the program since its launch with over 500 currently active.
There have been over 7000 home visits since it started and more than 350 referrals to Home & Community Care for further support.
