Grey County’s three long term care homes have reached the halfway mark of a three year initiative to implement the Registered Nurses Association of Ontario Clinical Pathways program, aimed to improve resident care through proven best practices.
Team members have been rolling out foundation tolls since April 30th 2024 which included admission assessment, delirium assessment, resident and family centered care, fall prevention and management of pain.
Jennifer Cornell, Director of Long Term Care and Senior Services commended the efforts saying they are proud of the teams commitment and they are building on the dedication to person centered care, reaching this halfway point shows the teams commitment to providing the best possible care for resident.
